We would ask patients, where possible, to come to the surgery between 9.30am and 5pm to register to avoid the busy peak times at the start and end of the day.
Your registration will be with the partnership. However, you have the right to express a preference for an individual GP. You may indicate this on the ‘application to join form’. However, please be aware that some of the GPs only work part time and not all of the GPs provide every service you may require. Your request, if made, will be entered onto the computer registration template as your preferred doctor.
Please complete a GMS1 form and New Patient Questionnaire before coming to the surgery:
Please try to obtain your NHS number before
registering, as this will make the registration process much easier.
You can find this on your NHS medical card, or alternatively your
current GP will be able to give it to you. You will also need to
bring proof of your address to the practice when you register such
as a tenancy agreement or utility bill.
If you have never registered with a GP in the UK before, we would
need to verify your identity before we can register you. Please
contact reception to find out what forms of ID are accepted.
